Unpaid IDR Awards: Payment Deadlines & Follow-Up

A favorable determination is not collected revenue. Match the award to the claim, calculate the remaining amount and follow the payment through the remittance and deposit.

When is an IDR award due?

For federal IDR, payment required by the determination is due within 30 calendar days of the payment determination. Calendar days include weekends; do not substitute a 30-business-day clock. State awards and negotiated settlements should be tracked under their own governing rules or terms.

    Calculate what remains unpaid

Read how the determination expresses the selected amount. Reconcile the total out-of-network rate, permitted patient cost sharing, the payer’s initial payment and any later payments. Do not demand the full selected amount again when part of it has already been paid.

Illustrative reconciliation: if the selected total rate is $5,000, applicable patient cost sharing is $500 and the payer already paid $1,000, the remaining payer amount is $3,500. This example assumes no other adjustments or payments and does not authorize changing the patient’s permitted cost sharing.

A reprocessed EOB is evidence of claim adjudication, not proof that funds arrived. Match the EFT or check trace to the bank deposit and the affected service lines. Keep fee refunds separate from claim reimbursement.

    Send a precise payment follow-up

Identify the dispute reference, claim number, determination date, selected amount, prior payments and remaining balance. Include the determination and a short reconciliation. Ask the payer to confirm the payment date and trace number, or to identify the specific reconciliation item it disputes.

Log each response and any partial payment against the same matter. If multiple claims were batched, a single payment may cover only some lines; update the line-level ledger before closing the case.

    Document noncompliance for CMS review

CMS provides a provider billing complaint channel for suspected noncompliance with the federal IDR process. Assemble the determination, claim/payment crosswalk, remittances, unpaid balance and payer correspondence. Follow the current portal’s documentation and redaction instructions.

Save the complaint confirmation number and attach later payment evidence to the same record. A complaint starts a review; it does not itself prove collection or guarantee a payment date. If administrative follow-up remains unsuccessful, obtain advice about the available enforcement options for the particular matter.

Close the case on cash and evidence

Keep separate statuses for determination issued, payment due, payment received, payment reconciled and escalation pending. Close the financial task only after the remittance and deposit support the amount credited. Record any unresolved shortfall separately from an unrelated payer recoupment.

Check the current official guidance and claim-specific facts before filing. MedRes can review the plan, service category, routing and supporting records with your team.
