UnitedHealthcare IDR: Plan Names, ID Cards & State Routing

Start with the state. Then identify who funds the plan and which services are in dispute. A UnitedHealthcare network name is a useful clue, but it cannot decide the payment dispute process by itself.

01 / Check your state first

    Where were the services furnished?

Start here, then confirm the policy state, funding, service category and date. This is a screening guide; selecting a state does not establish jurisdiction.

    02 / Identify the plan

UnitedHealthcare plan names and likely routing

Use these names to narrow the document review. A likely route still depends on the state and the specific claim.

03 / Read the funding disclosure

    Same network. Different funding.

These are Surest’s publicly published demonstration cards, not patient records. Both show the Choice Plus network. The funding disclosure on the front is the important distinction.

    Self-funded sample

Surest official self-funded sample member ID card, front and back, with UnitedHealthcare Choice Plus network
The front says “Self-insured coverage.” Check the employer plan and any applicable state opt-in.

    Fully insured sample

Surest official fully insured sample member ID card, front and back, with UnitedHealthcare Choice Plus network
The front says “Fully Insured” and identifies an underwriting field. Check the policy and state payment rules.

Source: Surest member ID card guide. Card layouts and wording vary. Confirm current coverage through the plan administrator.

Confirm the plan behind the card

Keep the front and back of the member card, the EOB or remittance, the Summary Plan Description and any written funding confirmation together. Record the employer or plan sponsor, legal payer name, administrator, product, group number and service date. A network logo, PPO label or payer ID does not establish who bears the claims risk.

Ask the administrator: Is this particular group fully insured, self-funded or level-funded for the date of service? Which entity is responsible for payment? If state opt-in is available, was this plan enrolled for the relevant period? Retain the answer with the underlying documents.

Check the service before initiating a dispute

Federal IDR concerns the payment amount for qualifying out-of-network emergency services, certain non-emergency services at participating facilities, and out-of-network air ambulance services. Routine out-of-network office visits and scheduled care at an out-of-network facility do not automatically qualify.

Read the denial reason. A disagreement about coverage or medical necessity follows the applicable appeals process. A notice of denial of payment can still involve an eligible payment dispute; the label “denied” alone is not enough to classify it.

    Build a UnitedHealthcare dispute file

Match each claim to its actual plan and administrator before grouping work. Two cards using Choice Plus can represent different employers, funding arrangements and legal counterparties. Keep state cases and federal cases in separate filing queues, with the applicable deadline attached to each claim.

Preserve the payer’s current dispute contact instructions and proof of delivery for open negotiation. A routine reconsideration request should not be assumed to satisfy a separate federal notice requirement. Once an award is issued, reconcile the determination with the original payment and later remittances before recording collected recovery.

    • Check the state and service-date rules.
    • Confirm funding and any applicable opt-in.
    • Verify protected service category and network status.
    • Identify the proper party, notice channel and filing window.
    • Track the determination through actual payment.

Check the current official guidance and claim-specific facts before filing. MedRes can review the plan, service category, routing and supporting records with your team.
